In this patient-centered, mission-driven role, you will optimize safe, evidence-based medication therapy across inpatient and/or ambulatory settings. Responsibilities include verifying orders, performing medication reconciliation, providing pharmacotherapy consults, and educating patients and clinicians. You will collaborate in interdisciplinary rounds, support antimicrobial stewardship and quality initiatives, and contribute to protocol development.
