About the Role:

Join VITAL HOME CARE AGENCY LLC as a Home Care Private Duty RN/LPN in Phillipsburg, NJ! We are seeking dedicated nursing professionals to provide exceptional care to our clients in the comfort of their homes. Responsibilities:

• Provide 1:1 compassionate and personalized nursing care to clients in their homes.
• Administer medications and monitor patient health status.
• Assist with daily living activities, including hygiene and mobility support.
• Educate clients and families on health management and wellness practices.
• Document patient progress electronically and communicate changes to the care team
• Ensure a safe and comfortable environment for clients.

Requirements:

• Current RN or LPN license in the state of New Jersey.
• Previous experience in home care or a similar healthcare setting.
• Trach and/or vent experience preferred 
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate demeanor with a commitment to patient-centered care.
• Reliable transportation and willingness to travel to client locations.
• Flexible availability for full-time or part-time shifts.
